What is the Medicare GUIDE Model?
The Guiding an Improved Dementia Experience (GUIDE) Model is an innovative Medicare initiative developed to enhance quality of life for dementia patients, lessen caregiver burden, and enable individuals to remain safely in their homes rather than transitioning prematurely to nursing facilities.
Through an interdisciplinary care team, JNH Home Health Services provides an all-encompassing support system tailored specifically to the evolving challenges of dementia and Alzheimer's disease.
Who is Eligible for GUIDE?
- ✓Enrolled in Traditional Medicare Part B (not enrolled in Medicare Advantage HMO/PPO plans).
- ✓Diagnosed with dementia or cognitive impairment (mild, moderate, or severe).
- ✓Living in a community setting or private home (not residing in a long-term skilled nursing facility).
- ✓Not currently receiving Medicare hospice benefits.

You never have to navigate dementia care alone. A dedicated care coordinator is assigned to your family to facilitate all aspects of support.

Is there any out-of-pocket cost for the patient or caregiver?
For beneficiaries with Traditional Medicare Part B and secondary supplemental coverage (Medicaid or Medigap), services under the GUIDE program including care navigation and respite funding are typically covered with zero out-of-pocket costs.
Can I keep my current primary care doctor while enrolled in GUIDE?
Yes! The GUIDE program does not replace your primary care physician. Rather, our care navigators and medical oversight team collaborate directly with your existing doctor to ensure unified, coordinated dementia management.
How can the $2,563 annual respite care fund be utilized?
The respite stipend can be used towards in-home respite aides, adult day care programs, or short-term stays in community facilities, giving unpaid family caregivers the break they need to avoid burnout.
How does this work with Illinois Community Care Program (CCP) or DRS?
GUIDE operates alongside existing home care benefits. JNH Home Health Services coordinates both state programs (CCP/DRS) and Medicare GUIDE to maximize the in-home care hours and specialized support available for your family.
